If the certificate holder needs to be an additional insured, please check the box. If you need multiple certificates or AIs, please provide the name and address of each one.

Remember, in most cases, to add an additional insured for no charge, there must be a written contract or agreement requiring it. This applies, generally, if you have blanket additional insureds on your policy. If there is a charge or additional premium due, we will contact you to discuss the options prior to adding the additional insured.
